About Kaitlin: Kaitlin is a Native Vermonter, going back many generations. She attended UVM, receiving a BS in
Biology, then moved to Miami, FL to complete her graduate medical training. She realized VT would
always be her home, and moved back in 2008. After working in a surgical subspecialty for the past 15
years, she joined NOTCH in 2022, as family medicine has always been near and dear to her heart.
She is invested in taking care of patients by developing strong relationships based on integrity and
focusing on patient education. Her clinical interests include pelvic floor dysfunction, interstitial cystitis,
and preventative care. Outside of her career, she loves being a mom to her tiny humans, dogs, and
chickens. In her free time, she enjoys anything outdoors, playing cards, and crafting.

About Sarah: Sarah DeSilvey has had the honor to work at Georgia Health Center (GHC) for a decade. She practices full-spectrum rural family practice, pediatrics to geriatrics, and takes care of many 4 generation families. She went back to school to get her doctorate in health systems leadership and quality and loves taking care of patients, and working to take care of patients better. When she is not at GHC she leads a national collective charged with developing the language and data standards to address the social determinants of health.

About Therese: Dr. Ray is a board-certified family medicine physician who has long held an interest in health care, serving underserved communities in a healthcare setting in Nicaragua, Greece, CO, and MA before completing medical school at the University of Vermont.  Dr. Ray completed a Family Medicine residency as part of the UVM health network in Plattsburgh and received the AFMRD award for patient advocacy, recognized consistently for her strongly compassionate and patient-centered approach to care. Dr. Ray enjoys caring for people across all of life’s stages, though has a special interest in preventative health, pediatrics, adolescent medicine, reproductive health, and mindfulness. In her free time, Dr. Ray can be found outdoors hiking, biking, or enjoying her newest hobby, aerial dance, for fun and fitness.

About Nate: Nathaniel Burns PAC grew up in rural Maine. He has a bachelor’s degree (cum laude) in music and philosophy. During undergrad, he worked at a nursing home, and he enjoyed this job so much that he decided to pursue a career in medicine. Nate moved to Vermont to complete a pre-medical certificate at UVM and then went to Northeastern University in Boston for his master’s degree. He started at NOTCH in the Richford office in 2013 and transitioned to the Georgia office in 2022. Primary care is his calling; he values long-standing relationships with patients and their families. His clinical interests include healthy eating habits, the gut microbiome, geriatrics, and critical appraisal of medical research. In his spare time, he enjoys being with his family, playing with his dogs, mountain biking, snowboarding, traveling, and playing music.
