How to become a NOTCH Patient

Step 1: Find your Provider and Location

  1. Review our Medical and Dental locations and Providers to select your Provider of choice
  2. Call the Medical or Dental clinic where you want to establish care to schedule an appointment.

Step 2: Complete your Paperwork

  1. Complete the Medical New Patient Packet or Dental New Patient Packet and fax, mail, or drop off in person the completed forms. You can view our Dental or Medical location information for phone, fax, and mailing addresses.
  2. If you are establishing care in one of our Primary Care clinics and are on Medicare, please fill out and return to the office the Medicare Questionnaire
  3. If you are establishing care in one of our Dental clinics, please also return the adult or pediatric Dental History Questionnaire.

Step 3:  Financial Assistance

  1. If you need financial assistance, visit our Financial Assistance page to review options and fill out our Sliding Fee Application form.
