Care Management

The goal of NOTCH’s Care Management program is to help you meet your own goals for improved health and wellness.  Enrolled patients will be assigned a Registered Nurse Care Coordinator – a single point of contact at your clinic – who can help you with a variety things:

  • Medication Management
  • Symptom Management
  • Scheduling Appointments
  • Transportation to medical appointments
  • Affordability of medication
  • Obtaining necessary medical equipment etc.
  • Communication to and from your Primary Care Provider

Screening and Prevention - You First!

Northern Tier Center for Health works in partnership with You First Programs.  You First (formerly Ladies First) offers personalized support to pay for and connect eligible Vermonters to breast, cervical and heart screenings, diagnostic tests and heart healthy lifestyle programs. Members can get free memberships to WW® (Weight Watchers), TOPS® and local gyms, farmers’ market coupons, state park passes and more.
